The Client Assessment is a DSS reportable record which sits under the Session>Session Participant>Client Assessment.
There are three different types of Outcomes measured through SCORE which help tell the story of what has been achieved for individual clients;
Changes in client circumstances in relevant outcome domains
Progress in achieving client goals set within relevant goal domains
Client satisfaction with the contribution of the service to meeting their needs
SCORE is reported using a simple five-point rating scale. This provides a consistent and comparable way to translate outcomes into a quantified measure. It is intended that SCORE is recorded towards the beginning and end of a case within a funded activity, to best assess change pre- and post- service delivery.
ProntoCase assists you to easily create the regular Client Assessments for Client Circumstance and Client Goals by having a button on the Session Participants which allows you to create both records at once. Refer to this article.
DSS states that;
"While it would be valuable to have pre- and post- SCOREs for all clients, as well as progress SCOREs at review points in between, it is recognised that there are a range of situations where it is not feasible to collect the ‘ideal’ outcomes data. Despite best endeavours, there will be some occasions where pre- and post- outcome SCOREs may not be able to be recorded for each client that receives a service."
